Chronic Disease

The Piscataquis Public Health Council is involved in many supportive initiatives related to Chronic Disease. Our work in this area is grouped under two categories:
- Early Identification and Treatment and Access to Self Management Supports.
- Early ID and treatment services include providing materials to increase awareness of chronic diseases such as colorectal cancer, heart attack, stroke, asthma, and diabetes.
PPHC partners with community agencies who offer educational programs on these issues; and assist local business to develop emergency response plans in case of employee heart attack or stroke. We are also active in promoting efficient access to self management supports. We support:
- the 211 program, which connects people who want to give help or get help with a full range of health and human services in their community,
- we help business increase employee self awareness of chronic disease through materials dissemination,
- we assist in coordinating community awareness campaigns,
- and we have an online Physical Activity Guide that gives you great access to physical activity resources town by town.

Mayo Regional Hospital’s Cardio Pulmonary Program (Asthma, Lung Disease) is a full-service department offering a variety of testing and education programs for the inpatient and outpatient population with heart and lung problems. Mayo Regional Hospital’s Diabetes and Nutrition Center provides group classes and education about type I and type II diabetes, education for women diagnosed with gestational diabetes, medical nutrition therapy, and support groups for persons with diabetes or living with someone with diabetes. www.mayohospital.com/depart/dept_nutri.php
Maine’s Cardiovascular Health Program works to prevent death and disability from heart disease and stroke in Maine. We promote a way of life that supports and includes preventing and controlling risk factors, especially high blood pressure and high blood cholesterol, and increasing timely care for heart attacks and strokes. maine.gov/dhhs/boh/hmp/mcvhp/
